While many of the (larger) hearing aid companies make (or have made form them) custom chips, there is still a large market for standard hearing aid ICs and parts. I can't say how they compare to the full custom chips, but I would imagine the large companies go custom for more than just the price savings. Google "dspfactory" and "Gennum".

There are also some useful Yahoo Groups and some interesting finds on eBay. For your own situation, you can also buy non-custom hearing aids that are a standard mold, but have a foam cover for the "fit". The important part is the adjustment for your hearing (the eq and compression/gating). I have a bunch of links somewhere but apparently not on this computer.

If you wanted to make something, I would recommend an external amp/eq (I think they are called "body hearing aids" or some such thing). It could look like an iPod and hence you wouldn't be shunned in public. You could use an 8051 to control via IIC a device such as a TI TAS3002 (or similar). You'd then only need some audio i/o amps -- mic preamp (also available as a chip, see maxim) and a headphone amp. Getting it to sound good would be the trick.
