We generally provide remote monitoring software/hardware solutions. For your requirements, I can suggest 3 methods

1. Use Landline DTMF. Control from outside with a landline or mobile(if your GSM modem and service provider supports DTMF).

2. Use Mobile and SMS. We sell a product that does this. GSM modem at home will receive the command, will decode it and control outputs. You can use GPRS modem and TCP/IP.

3. You can use an ethernet I/O card at home and assign a public IP address to it. From any where else, you can connect to net and control the I/Os using a simple application. We sell a product like this.

You can also keep your PC on at home, connected to internet/telephone/GPRS modem and connect to it from a remote location and control your devices. We had attempted remote controlling through yahoo messenger to beat the constraint of having public IP.
