I'd point out that what often occurs is that the tantalum cap catches fire. This is not a happy circumstance, and certainly is alarming, in more ways than one, as they produce thick black smoke and lots of it.

BTW, I've never seen this happen when the power supply wasn't a switcher. Maybe it has to do with the fact it's a switcher, or maybe it has to do with the fact that I haven't seen it happen more than a dozen times. It's hard to form good conclusions on small samples.
