Here is my PCB layout in this order: TOP, IN2, IN15, BOTTOM and schema.

There is the switcher on the BOTTOM layer. There is also the output cap C28 on the top of the BOTTOM layer image. Vias at its sides connect GND (on the right) and Vcc (on the left) to the inner planes. It is shown on the layer IN2 and IN15.

There is the rectifier D10 on the bottom of the BOTTOM layer image. I placed there the FBs L4, L5 on the input of the switcher as you suggest. The input and output caps are on the same plane that is on the TOP layer. But there are also switcher gnd planes on the layer IN2 and IN15 and I do not know if I can connect them also together. Now they are separated. It is shown on the IN2 and IN15 image. I terminated them close to the GND of the output cap... Should I make the IN2 and IN15 switcher gnd plane same as the TOP switcher GND plane is (without the interspace)? Thank you very much...

I made switcher polygons in the same way as is shown in the LM25574 datasheet.
