Put the filter nearest to the connection of both ground planes! Best right at this connection!

By your methode you contaminate a large area of analog ground plane by the unavoidable digital ground return currents, which "survive" the filtering. Also, your design forms a too large loop area.

Use a pi-filter with each cap on each side of the connection between both ground planes, traversing the "gap" by the inductance (ferrtite) only.
