That does depend on what the load is.

If the load is some form of respiratory equipment, the fail-safe alternative may be to run the equipment.

The important factor here isn't if an output is active or not when the processor isn't powered, or if the processor is in reset.

The really important factor is that the product owner should define beforehand what the default states should be. It should not be something that just happens because of the selected solution, and is later just documented (or not...) in the product manual. To be able to validate a solution, there must first exist detailed requirements.
