I linked the application note from the On Semiconductor web site in this previous posting:

One has to be careful with linking documents, data sheets and application notes from stray web sites. Sometimes these change!! John - the app note copy that you linked is Rev 3 of April 2002. On the other hand the version I linked direct from On Semiconductor's site is Rev 5 of May 2006. (Over the years this app note has been known to have mistakes in it and one would do well to use the later version. I think there are still some mistakes still in the Rev 5. :-) )
