Sorry, that link was because of where I found your email. You can contact sales or tech support at my company (see link below) and see if they will sell you your desired quantity of our 5 x 4 matrix PCB. It has cherry switches and a SIP header, I don't remember if it had diode-per-key or not. This is a pic of it in someone's product, but I don't have a pic of the PCB:

The keycaps are a separate issue. You can ask if we have the keys you want (it's possible), make them yourself using relegendable keycaps, or get them custom made.
