Thanks a lot for your valuable feedbacks, as being a hobbyist i didn't paid much attention to all these problems but now i know the severity of problem so will improve the said things
i have noted all the feedbacks and will go for new layout of board,
will definitely update you all with the new version ,will take some more time as a chained process

a summary of points goes like this

debounce
faraday shied
remove cap at o/p
shielded cable
protection diodes
solid ground plane
many wires different locations wrong
all connectors at one edge
use shieded cables
ground them at entering point
