That is easy.

Because a huge number of organisations thinks that ordering a product is an economical thing, so the important people comes from the purchase department and do not know any technology.

And them being project managers, they also manage to order the requirements specification from someone they have a good contact with - someone closer to economy than to technology. So still more economoy or sales people gets involved. And they bolster their collective minds with how skilled they are and how cunning they make their contracts to make sure they will get a good price.

Ten months later, you stand there with your product and find out that they ordered a 230V product for use in a country with 110V or analog video output to a device only having digital video inputs.

It's all huge factor of lack of communication. Too many people assuming that they know what is required, so no need to check with someone else.
