If the output needs to be isolated then you should be using a transformer! You can achieve the output voltage you need by winding the transformer with the appropriate turns ratio, i.e. 5:1 and using a forward converter circuit.

If the output doesn't need to be isolated, then you can use a flyback converter. This wont be as efficient as a forward converter, but it will be simpler. I would use a dedicated UC3845 controller.
