I have made the new circuit board and tested it, and everything is now working as it should.

I used Michael's circuit with a few alterations. I replaced Q2 and Q3 with a TIP120, put a 1k resistor in place of the 15 ohm resistor, and omitted the 150 ohm resistor (since the TIP120 has built-in base-emitter resistors, as Michael mentioned).

Thanks to everyone for the help in resolving this issue.
