It could very definitely be a failure if the program loader code was not carefully designed and fully validated!!
How often have you seen last minute, seat of the pants, features added that never get the full rigor of the design and validation cycle? I've witnessed this happening on many projects over the years and many of the critical bugs in those projects end up being introduced by those last minute additions!! Michael Karas |
