Some things to look really hard at...
- Does your display module happen to be one of the odd ones that has two controllers on it and supporting possible two E strobes such as E1 and E2? - Have you simply tried to write in all 80 locations of the display controller memory with a incrementing alphabet pattern to see if there is some oddity with the display controller column drivers being mixed up in a weird order? - Have you thought about to post and specify the module manufacturer and part number or point to a web site with detailed data sheet so maybe an LCD module expert here could look to see if they spot anything odd.
